List of necessary documents
For opening of account in securities investment manager provides the following documents:
- application for opening of account in securities
- blank of account in securities
- blank of manager of account in securities
- copy of certificate of state registration of investment fund and investment manager, attested by a notary or by body that issued certificate of state registration
- copy of certificate about registering of investment fund and investment manager into single state docket of enterprises and organizations of Ukraine, attested by a notary or by body that issued this certificate
- copies of registered constituent documents of investment fund and investment manager, attested by a notary or by body that accomplished registration
- copies of documents certifying appointing on position a person, that has right to act on behalf of investment fund and investment manager without letter of attorney
- original or copy of letter of attorney of account in securities manager, issued and signed by head of investment manager and attested with a seal of investment manager, if manager of account is not a head of investment manager
- original or correspondingly attested copy of document, containing information about settlement account and essential elements of the bank, where settlement account of investment fund is opened
- card of samples of account in securities managers signatures and press of seal of investment manager, attested by head of investment manager
- copy of agreement with investment manager about managing investment fund
- copy of license on conducting professional activity on stock market - activity of trade with securities, issued to investment manager
- copy of investment declaration and information about issue of investment certificates, registered by GKCBFR
- list of constituents and other persons of investment fund, attested by investment manager
