To open account in securities to pension fund the person that manages the assets of pension fund provides the following documents:

  1. application for opening of account in securities
  2. blank of account in securities
  3. blank of manager of account in securities
  4. copy of certificate about state registration of pension fund, attested by a notary or by body that issued copy of certificate
  5. copy of certificate about state registration of financial establishment, issued to pension fund, attested by a notary or by body, that issued certificates about state registration
  6. copy of certificate about state registration of Manager of account in bodies, authorized to accomplish state registration, attested by a notary or by body that issued certificates about state registration
  7. copy of certificate about registration in Single docket of enterprises and organizations of Ukraine, attested by a notary or by body that issued this certificate
  8. copy of registered statute of pension fund, attested by a notary or by body that accomplished this registration
  9. copies of documents certifying appointment on position persons that have right to act on behalf of pension fund without letter of attorney
  10. copy of registered constituent document of manager of account, attested by a notary or by body that accomplished this registration
  11. copies of documents certifying appointment on position persons that have right to act o behalf of manager of account without letter of attorney
  12. original or copy of letter of attorney of manager of account in securities, issued and signed by head of manager of account and attested with a seal of manager of account, if manager of account is not head of manager of account
  13. copy of agreement about managing the assets of pension fund
  14. copy if license on conducting professional activity on stock market - activity on management the assets on institution investors activity on managing the assets)
  15. copy of investment declaration
  16. original or correspondingly attested copy of document, containing information about settlement account and essential elements of the bank, where settlement account of investment fund is opened
  17. card of samples of account in securities managers signatures and press of seal of investment manager, attested by head of juridical person
